Chronic Urticaria - Diagnosis and Treatment Algorithm

הגדרה וסיווג - Definition & Classification

Chronic Urticaria (CU): Wheals ו/או Angioedema ≥6 שבועות. חלוקה:

  • CSU (Chronic Spontaneous Urticaria): ללא טריגר מזוהה; ~80% מהמקרים
  • CIndU (Chronic Inducible Urticaria): טריגר פיזי - Dermatographism, Cold, Cholinergic, Solar, Pressure, Vibratory

CSU היא מחלה אוטואימונית ברוב המקרים: Type IIb (Anti-IgE או Anti-FcεRI autoantibodies) או Type I (IgE-mediated autoallergy).

💡Clinical Pearls

  • •Wheals שנשארים >24 שעות + Purpura/Residual hyperpigmentation + כאב > גרד = חשד ל-Urticarial Vasculitis. ביופסיה חובה.
  • •אין צורך בבדיקות אלרגיה (Prick/RAST) ב-CSU - זו לא אלרגיה קלאסית.

אלגוריתם טיפולי - Treatment Algorithm (EAACI/GA²LEN 2022)

Step 3Omalizumab 300 mg SC q4wAdd-on לAH. תגובה ב-65-80%; מינון 150-300mg
Step 4Cyclosporine 3-5 mg/kg/יוםAdd-on ל-Omalizumab שכשל; Short-term (3-6 חודשים)

💡Clinical Pearls

  • •Up-dosing לx4 = קו שני לפני Omalizumab. רוב המטופלים לא מודעים שאפשר להעלות מינון. Bilastine ו-Fexofenadine - פחות Sedation ב-Up-dosing.
  • •Omalizumab responders מתחלקים ל-Early (שבועות 1-4) = Type I, ו-Late (שבועות 8-16) = Type IIb. אם אין תגובה ב-6 חודשים = Non-responder.

הערכת חומרה ומעקב - Severity Assessment

UAS7 (Urticaria Activity Score 7): מדד שבועי. ציון wheals (0-3) + גרד (0-3) × 7 ימים = 0-42. <7 = מבוקר; 7-15 = קל; 16-27 = בינוני; 28-42 = חמור.

UCT (Urticaria Control Test): שאלון 4 שאלות. ≥12 = מבוקר. מטרה: UCT ≥12 ו-UAS7 ≤6.
